Great Lakes PACE PACE is a program for adults 55 & older. We provide all-inclusive care for seniors with disabilities, helping them remain living in the community.

WHO CAN JOIN?
• You are 55 years old or older.
• You live in the service area of a PACE organization.
• You are certified by the state in which you live as meeting the need for the nursing
home level of care.
• You are able to live safely in the community with the help of PACE services. SERVICES
• Primary Care (including doctor and
nursing services)
• Hospital Care
• Medical Specialty Services
• Prescription Drugs
• Nursing Home Care
• Emergency Services
• Home Care
• Physical therapy
• Occupational therapy
• Adult Day Care
• Recreational therapy
• Meals
• Dentistry
• Nutritional Counseling
• Social Services
• Laboratory / X-ray Services
• Social Work Counseling
• Transportation
PACE also includes all other services determined necessary by your team of health
care professionals to improve and maintain your overall health.

Medicare’s Annual Enrollment Period (AEP) runs from October 15 to December 7 each year. During this time, people with Medicare can review, change, or enroll in Medicare Advantage (Part C) or prescription drug (Part D) plans for the upcoming year. Insurance companies advertise heavily during this time and it can be confusing!!!

We make senior healthcare easier for you! No deadlines, no confusing Parts. Great Lakes PACE is all-inclusive senior healthcare!

Great News!!! When you qualify for Great Lakes PACE program, you don’t NEED Part C or Part D because everything is already provided in our FREE senior healthcare! And if you qualify for PACE, you can enroll at any time during the year! We make senior healthcare easier for you!

Life can be so lonely, especially as we age.

A key benefit of PACE is the adult day health center which offers social activities, recreation that is also physically therapeutic, nutritious meals enjoyed with others, and opportunities to interact with friends and staff.

Great Lakes PACE has a wide range of activities like arts and crafts, games, music therapy, exercise classes all tailored to the needs and interests of people 55+.

To qualify for PACE and our FREE Senior Healthcare:
1. 55+
2. Have both Medicare and Medicaid
3. Need help with daily living. We'll help you assess this!
