DE QUERVAIN'S TENOSYNOVITIS

De Quervain's tenosynovitis is an inflammation of the tendons on the thumb side of the wrist, resulting in swelling, tenderness, and difficulty with pinching, grasping, or making a fist.

What Causes ir?
• May be caused by overuse of the thumb and wrist.
• Is also associated with pregnancy and breastfeeding.
• Rheumatoid arthritis patient are more susceptible.
• Most common in people in their 40s and 50s and affects more women.

What are the Symptoms?
• Pain over the thumb side of the wrist. Pain starts in the wrist
and can travel up the forearm. The pain is usually worse when
forcefully grasping and/or lifting objects or twisting the wrist.
• Swelling over the thumb side of the wrist.
• Catching or snapping sensation when moving the thumb.
• Difficulty moving your thumb and wrist.

Treatments available:
• Splints. A removable splint to keep the wrist straight and the
thumb still may improve pain, especially when worn at night.
• NSAD. Drugs like ibuprofen and naproxen can be taken by
mouth or applied topically. They may help reduce swelling and
relieve pain.
• Avoiding activities that cause pain and swelling.
• Corticosteroids injection into the tendon sheath can help.
However repeated cortisone injections may make it worse.
• Platelet Rich Plasma (PRP) Injection is the best option now a
days and you may be able to avoid surgery. It can address the
pain from the tenosynovitis and the pain coming from inside the
wrist joint!
• Surgical treatment. A last choice. The goal of surgery is to
release the tendon sheath to make more room for the irritated
tendons.

YOU HAVE PROBABLY HEARD THE SAYING THAT GOOD THINGS COME IN PAIRS! BUT DON’T FORGET THAT BAD THINGS DO TOO. AND AT TIME MORE FREQUENTLY!

In the last three month I have had three female patients fifty years old and up, come to the office complaining of hip pain. In one it was bilaterally. They had been dealing with the discomfort for a long time and have had cortisone Injections, physical therapy, rest, application of heat etc. with no relief. None of the three have had an MRI ordered before visiting our office. We did order hip MRI’s in all three patients. In two we found labral tears and in all of them we found one or more gluteal tendon tears. This tells you how important a good detailed history and physical exam are to guide you as to what test to order. They all have had x-ray that show nothing was wrong and in one of them their insurance refused to pay for the MRI and the patient had to pay out of pocket!

So far, the first of the three patients has been treated successfully with high dose PRP using guided injections under local anesthesia in our fully equipped procedure room. Gluteal tendon tears are common in women, particularly those aged 50-70, and are often caused by chronic degeneration rather than acute injury. So, if you have hip pain don’t assume you need a hip replacement. A simple in office injection with high dose PRP may do the trick for you!
