Getting to sit down with Joycelyn Elders (the 15th US Surgeon General) in the New York Academy of Medicine Library and talk about women’s healthcare especially what’s happening (or not happening) in perimenopause and menopause.

Because here’s what I see every day:

I just completed an advanced conference on breast cancer and hormone therapy, and it reinforced something I see every day:

A history of breast cancer does not always mean HRT is off the table.

But here’s the truth…

👉 Hormones are nuanced
👉 The type, timing, and delivery all matter
👉 And the provider guiding you matters just as much

This is not protocol based care.

There’s something powerful about being in a room with clinicians who are PUSHING medicine forward.

A4M has been shouting from the rooftops for DECADES and this year screamed gut health and the connection to chronic disease, cardiovascular disease, immune dysfunction, hormone regulation and more.

This past week at A4M Spring Congress in West Palm Beach, I had the opportunity to learn from and connect with some of THE most respected voices in hormone, gut, immune health and the integrative medicine space.

And one message echoed again and again from multiple experts:

👉 You are not just as strong as your hormone health…
👉 You are only as strong as your gut health.

Because your gut influences:
• Hormone metabolism
• Immune regulation
• Inflammation
• Brain function
• Even how you respond to treatment.

I also had the chance to learn from leading voices integrative cardiovascular medicine with Dr. Pradip Jamnadas (who is hilarious and witty all in one) reminding us that true hormone optimization must always include heart health, metabolic markers, and inflammation and I am excited to do a little more intermittent fasting! Such incredible pearls this weekend!

Truths about pellet therapy

1. It delivers consistent hormone levels
Pellets are inserted under the skin and release hormones steadily over time, which can reduce the “ups and downs” some women feel with creams or pills.

2. It’s convenient
Once inserted (typically every 3–4 months), there’s no daily remembering—huge win for busy women juggling everything.

3. It can be very effective for symptom relief
For the right candidate, pellets can significantly improve symptoms like fatigue, low libido, brain fog, and mood swings.

4. It bypasses the liver
Because it’s not taken orally, pellet therapy avoids first-pass metabolism, which can be beneficial for certain patients.

5. It uses bioidentical hormones
Most pellets contain hormones structurally identical to what your body produces (commonly estradiol and/or testosterone).

Lies (or at least…misleading claims)

1. “It’s the most natural and safest form of HRT”
Not exactly. “Natural” doesn’t automatically mean safer. Safety depends on dose, patient selection, and monitoring—not the delivery method alone.

2. “You’ll have perfectly balanced hormones every time”
Pellets are not adjustable once inserted. If the dose is off, you’re riding it out for weeks to months.

3. “No side effects”
That’s just not true. Possible side effects can include acne, hair growth, mood changes, or too-high hormone levels—especially with testosterone.

4. “It’s better than all other forms of HRT”
Pellets are one option, not the option. Some women do better on transdermal creams, patches, or oral therapy depending on their physiology and goals.

5. “You don’t need follow-up labs”
Huge red flag. Monitoring matters—symptoms and labs should guide ongoing care.

Pellet therapy isn’t “good” or “bad”—it’s patient-dependent.

I thought so many things were “normal” but within a few weeks of knowing my food sensitivities and changing diet gut issues resolved and joint pains disappeared.
