Clinical Clarity: Trajectory Tells the Story
One of the most common documentation gaps in hospice care isn’t the absence of data — it’s the lack of connection. Documentation often captures isolated findings, such as:
- A PPS score
- A symptom
- An ADL limitation
However, hospice eligibility is determined by trajectory, not mere snapshots.
Effective hospice documentation establishes a clear connection between:
- Where the patient was
- Where the patient is now
- How today’s findings indicate ongoing decline
When documentation consistently illustrates change over time, the medical record conveys a clear and defensible clinical story, essential for care planning, interdisciplinary communication, and compliance.
Clarity isn’t about writing more; it’s about writing with purpose.
