One of the simplest ways to strengthen a hospice chart—and one of the most overlooked—is learning to document what you actually see, not what you’ve gotten used to writing.

- “No new concerns”

One of the most common survey findings in hospice is discipline mismatch. Surveyors don’t just read nursing notes; they compare them against MSW and chaplain documentation to see whether the entire team is describing the same clinical reality.
