Trust Your Gut – You Need More Sleep

Modern sleep research begs the question; Is there any health benefit to poor sleep? Seemingly every new bit of research confirms the obvious answer. The latest comes from researchers at the University of Illinois Chicago who found associations among disrupted sleep, elevated blood pressure, and changes in the gut microbiome. The results were published in Physiological Genomics.

“We know that working at night can cause problems with your health, and the data suggest that staying awake all night can lead to high blood pressure, and, in some cases, eventually to heart disease, but it’s not clear what mechanisms underlie the development of these conditions,” said Anne M. Fink, assistant professor, UIC College of Nursing’s Department of Biobehavioral Nursing Science.

Researchers found that the gut microbiome changes did not happen immediately, but instead took a week to show unfavorable responses such as an imbalance among different types of bacteria, including an increase in microbes associated with inflammation.

Sleep medicine physicians know the benefits of proper slumber because they help patients and see the results. In recent years, turf wars have given way to a patient-centered culture that values options.

“There was a time when oral appliances for the treatment of obstructive sleep apnea [OSA] were not given the proper respect from those who believed CPAP was the gold standard of treatment,” says Shad Morris, D.M.D., owner of Premiere Sleep Solutions, St. George, Utah. “Now these same physicians understand that the problem of OSA is vast, and we need to adopt a philosophy that values real solutions.”

Science, Celebrity, and REM Sleep for Emotional Health

It has taken a while for mainstream media to catch up to non-traditional media outlets. The podcast platform wielded by Joe Rogan, for example, gets approximately 200 million monthly downloads by most estimates. It’s a staggering number, and when podcasts such as these tackle the subject of sleep medicine, it reaches a much different audience than a typical association press release.

Sandwiched between all manner of pop culture fodder, other shows such as The Peter Attia Drive include more science-based content. As a Stanford-trained M.D., Attia builds on his longevity-focused practice in Los Angeles to promote various new theories on healthy living.

On Aug. 31, 2020, Attia featured Matthew Walker, PhD, a professor at U.C. Berkeley who also appeared on the Joe Rogan Experience (JRE) in 2018. Commenting on the connection between emotional health and rapid eye movement (REM) sleep, Walker said that overall dreaming has increased during the time of COVID, perhaps a sign that people are working out their subconscious issues during slumber.

As a way to purge negative emotion and stress, Walker explains: “One of the principle functions of REM sleep is that it seems to provide a form of overnight therapy. REM sleep is emotional first aid, and it’s during REM sleep that we deal with emotional concerns, and even some of the traumatic events that we’ve been experiencing. REM sleep almost acts as a nocturnal soothing balm.”

While acknowledging that professional psychological counseling helps countless people, it’s undoubtedly less expensive to simply get great sleep. “In my sleep-centered practice, we offer oral appliances to address obstructive sleep apnea,” says Shad Morris, D.M.D., owner of Premiere Sleep Solutions, St. George, Utah. “We also offer positional sleep therapy in the form of slumberBUMP, an inflatable bladder that is comfortably worn during sleep, prohibiting sleepers from rolling onto their backs. CPAP Low Compliance is Not a Myth!!

Any rudimentary investigation of the research yields a familiar theme; continuous positive air pressure (CPAP) to treat obstructive sleep apnea is effective, but only about a third of patients consistently use the device.

One 2016 study published in the Journal of Otolaryngology – Head & Neck Surgery came to a sobering conclusion: “The rate of CPAP adherence remains persistently low over twenty years worth of reported data. No clinically significant improvement in CPAP adherence was seen even in recent years despite efforts toward behavioral intervention and patient coaching. This low rate of adherence is problematic, and calls into question the concept of CPAP as gold-standard of therapy for OSA.”

“The conclusion is fairly stark, considering that the study examined two decades years of data,” says Shad Morris, D.D.S., founder/inventor of the slumberBUMP positional sleep therapy device. “To truly embrace ‘total sleep wellness,’ clinicians need to be educated about oral appliances and positional sleep therapy, both of which tend to have higher compliance rates.”

Yet another recent study examined whether oral appliances worsened temporomandibular disorder (TMD) pain when compared to nasal CPAP. Yet again, oral appliances fared well with researchers concluding: “A low frequency of clinical signs of TMD pain in mild to severe OSA patients was found after 6 months, regardless of treatment with MAD [mandibular advancement devices] or nasal CPAP. In addition, no difference in mandibular function impairment was observed between the different treatment modalities.”

“Positional sleep therapy is a fairly low-tech option that patients can use immediately with no need for a prescription,” adds Morris. “It can be used in combination with the CPAP gold standard, by itself, or with an oral appliance to reduce snoring and improve the airway for patients.”

A survey by the American Academy of Sleep Medicine (AASM) confirmed the proposition, concluding that roughly 32% of Americans typically feel more tired than usual the day after election night. Later times on the east coast garner an “extremely tired” (12%) rating in the survey due to results that come later in the evening.

To achieve restful sleep on election night, and the days following the election as votes are tabulated, the AASM advocates that patients:
• avoid consuming caffeine six hours before your anticipated bedtime to ensure sleep is not impacted;
• avoid alcohol, because it can induce sleep and impact your sleep cycle and quality;
• watch election night coverage on a television outside of the bedroom to minimize blue light exposure and reinforce the bedroom as a space for sleep;
• keep your phone out of the bedroom to resist the temptation of social media;
• obtain a minimum of seven hours of sleep; and
• head outdoors the next morning, as sunlight exposure will help regulate your circadian rhythm.

The beginnings of Premier Sleep Solutions started when Dr. Shad L. Morris began observing the toll that obstructive sleep apnea (OSA) was taking on the oral health of his patients. A friend and dental patient whom he treated suddenly passed away in his sleep from obstructive sleep Apnea one night due to not wearing his CPAP.

Dr. Morris decided he wanted to help save lives by treating OSA. In taking many continuing education courses on OSA, along with the trained staff, Premier Sleep Solutions is now saving lives daily. His involvement in sleep medicine turned from primarily screening for sleep apnea to now having very close relationships with sleep physicians and allied health professionals in the community in treating this disease.
