Low back pain that just won’t go away? It might not actually be your “back.” 👀

One often-overlooked cause is a pelvic upslip.

A pelvic upslip happens when one side of the pelvis (ilium) shifts slightly upward compared to the other side. This can occur after a fall, sports injury, stepping off a curb awkwardly, or even repetitive movements over time.

Here’s why it matters:
🔹 When one side of the pelvis sits higher, it changes the alignment of the sacroiliac (SI) joints.
🔹 That uneven positioning creates tension in the surrounding muscles and ligaments.
🔹 The lumbar spine (low back) has to compensate for the imbalance.
🔹 Over time, this compensation can lead to stiffness, inflammation, and pain.

Common signs of a pelvic upslip:
✔️ Pain on one side of the low back
✔️ Pain that worsens with standing or walking
✔️ A feeling of being “crooked” or uneven
✔️ Tight hamstrings or hip flexors on one side

The good news? When properly assessed and treated, correcting the pelvic alignment and restoring muscle balance can significantly reduce low back pain.

✔️ Take a moment before getting up
✔️ Check for pain, dizziness, or weakness
✔️ Let your doctor or physical therapist know

The 3 Systems That Control Balance (And How PT Trains Them)

Balance isn’t just about “being careful.” It’s controlled by three key systems working together:

🧠 1. The Visual System
Your eyes help you understand where you are in space. Changes in lighting, uneven ground, or turning your head all challenge this system.

👂 2. The Vestibular System
Located in the inner ear, this system senses movement and head position. It’s crucial for staying steady when you move, turn, or look around.

🦵 3. The Somatosensory System
This includes muscles, joints, and nerves — especially in the feet, ankles, and hips — sending feedback to your brain about body position.

When one system weakens, the others have to work harder. That’s where physical therapy (PT) comes in.

How PT trains balance:
✔️ Strengthening legs and core
✔️ Challenging balance in safe, controlled ways
✔️ Training vision + head movement together
✔️ Improving joint mobility and body awareness
