We were honored to welcome Prof. Sunil Lakhani, a Professor of Pathology at the University of Queensland (Brisbane, Australia), to our San Antonio campus yesterday. A world-class leader in breast pathology, Prof. Lakhani has contributed extensively to advancing our understanding of breast disease. He is a member of the WHO “Blue Book” expert group and recently authored a chapter on breast pathology in Robbins & Cotran Pathologic Basis of Disease.

During his visit, we had an engaging discussion on mutual areas of interest including collaborative research, artificial intelligence in diagnostics, digital pathology, and strengthening resident education and training through shared academic initiatives.

From Polished Prose to Proven Science: Funding Brains, Not Bytes

Seed grants work when they back talent, sharp questions, and accountable milestones, not shiny prose. With generative AI making it easier to draft proposals, this commentary argues for investigator-centric funding plus clear disclosure and guardrails: AI can help with clarity and structure, but novelty, rigor, and feasibility must come from the investigator.

When Science Slips: The Hidden Cost of Misleading Data

In the high-stakes world of scientific discovery, I have come to see just how much each dataset shapes the story and how even a single error can echo far beyond the lab. In this editorial, Sohail Rao, MD, MA, DPhil, reflects on the consequences of misleading or manipulated data and how it can quietly distort the scientific narrative, misguide clinical decisions, and erode public trust.

But this isn’t just a reflection; it is a call to action. As researchers, reviewers, editors, and institutions, we share a collective responsibility to uphold the ethical foundations of academic publishing and protect the credibility of our work.

Invisible Risks, Unequal Burdens: The HPV Crisis in Rural America

While HPV is the most common sexually transmitted infection in the U.S., its consequences are not distributed equally. Women in rural areas face significantly higher rates of HPV infection, cervical cancer, and mortality.

In their compelling narrative review, Kari Hooper, MD, and Sohail Rao, MD, MA, DPhil, thoroughly examine the structural, cultural, and clinical factors that contribute to these disparities. From limited access to screening and vaccination to cultural stigma and geographic isolation, this article underscores the silent epidemic affecting underserved women across rural America.

Why should you read it? This paper not only highlights the problem but also offers evidence-based, community-centered solutions that can guide public health interventions, academic research, and policy reform.

AI, AGI, and the Evolution of Evidence in Clinical Decision-Making

In this timely article, Dr. Sohail Rao explores how Artificial Intelligence (AI), Artificial General Intelligence (AGI), and emerging technologies are being used to simulate, predict, and optimize clinical interventions. The piece critically examines AI's evolving role in clinical decision support, highlighting the ethical and regulatory boundaries that still require human oversight.

The article calls for a future rooted in hybrid models, where AI enhances efficiency, but human judgment preserves the rigor and reliability that evidence-based medicine demands.

Rethinking Clinical Trials in the Age of AI and AGI

Prospective interventional studies are the foundation of evidence-based medicine. However, this foundation is being questioned due to the rapid rise of Artificial Intelligence (AI) and the conceptual emergence of Artificial General Intelligence (AGI).

This insightful manuscript explores whether AI and AGI can augment or replace traditional clinical trials. It examines AI's current capabilities in trial design, decision support, and retrospective analysis while highlighting the ethical, methodological, and regulatory limits of depending solely on machines. AGI may promise more, but it also poses risks: bias, lack of transparency, and unclear validation pathways.

The future may lie in hybrid trial models, where human science and machine intelligence collaborate without compromising rigor.
