"This company is amazing. Cindy and her team have extensive knowledge on the ongoing care for the infirm and elderly. Our brother had a massive stroke at 61 yrs old, with a Thai wife who spoke little English and a two month old baby.

Cindy evaluated Gary and guided us through the whole process of facility placement, immediate healthcare concerns and finally ongoing in-home care upon discharge. She evaluates the needs of the patient and helps the family navigate through the complex and confusing healthcare system. More importantly, she can foresee problems before they arrive.

Cindy has experience with stroke victims in the past so her help was invaluable. She made recommendations that other family members rejected but she was always correct in her evaluations. Other family members were in denial but she helped navigate their reasoning back to sanity.

She also understands all aspects of elder care (like insurance issues), which is necessary in today's long term care environment. She truly cares about her patients and follows up at all hours of the day. I would hire Grace Care Management in the future should the need arise..."

Three things to remember when...

Your elderly loved one is falling through the cracks of the health care system:

1. If you are your family member's legal representative and you are not getting the responses and answer you need from you health care providers... keep asking. Call, email, fax.
Persistence is important!

2. If something is of concern to you and you are unsatisfied with the response from the front line office workers, ask to speak to the office supervisor or "case manager" in home health care or hospice agencies.
Run it up the chain of command.
Work arounds are important!

3. Many important things, sometimes even physician orders get missed, misinterpreted or just plain stuck somewhere in the process of moving between different levels of care. Ask for a copy of the discharge orders so you can follow up.
Connecting the dots is important!

Three things to remember when...

Your elderly loved one lives alone and you worry about them.

1. Help them install a safety alert system! There are many out there at very reasonable monthly fees. This will alert emergency personnel if there is a fall or other urgent issues needing immediate attention.

2. If that's not possible, call them to "check in" frequently. A call may not seem like much to you but to them it can be sunshine on their cloudy day. A huge neurotransmitter boost with no pharmaceutical side-effects.

3. If you are not able to visit, hire a caregiver or a geriatric care manager to go check in on them frequently if you cannot. It can be as little as a couple hours a few days a week. They can not only be companionship and a friend, they can make sure there's food in the refrigerator, that your loved one is taking their medications and most importantly, that they're safe.

Three things to remember when...

Your loved one has a sudden, significant change in mental status or unusual confusion:

1. Don't minimize it. Report this to the primary care physician right away.

2. Going to urgent care to rule out something serious is a good idea.

3. Sudden confusion is not necessarily dementia. There are many other reasons for an altered mental status. It's important to identify the source and treat quickly before things escalate.
