The Community Health Worker Supervisor Training equips supervisors and managers with the skills and knowledge to excel in their role and effectively lead their team of CHWs. Training lessons cover a range of topics, including effective communication, conflict resolution, and cultural competence.

Congratulations to Michella Meza, Care Transitions Supervisor within the Care Management Department, for receiving Employee of the Month!

Michelle has played a key role in the success of the Care Transition Choices (CTC) program since joining Partners in July 2022. She leads a team supporting patients discharged from Providence and Arrowhead hospitals, ensuring they are connected to the care and resources needed to remain safely at home.

Through Michelle’s leadership, the CTC team continues to deliver measurable impact, maintaining low readmission rates by prioritizing timely post-discharge follow-up, coordinating appointments, and addressing barriers to care. Her ability to translate process into outcomes strengthens both patient experience and program performance.

Michelle consistently looks ahead, identifying opportunities to streamline workflows, improve coordination, and leverage technology to make the work more effective and efficient. She leads with a clear focus on quality and results, ensuring her team is positioned to deliver at a high level.

Since joining Partners in June 2022, Rolando has provided steady leadership to the Community Wellness team, ensuring the successful delivery of evidence-based workshops across Los Angeles County. His work expands access to programs that help older adults maintain independence, reduce risk, and improve quality of life.

This year, Rolando was instrumental in co-leading a Spanish-language falls prevention workshop through a unique partnership with MyPlace Health, broadening access to critical prevention services. He also helped research and pilot two new program series, Wellness Initiative for Senior Education and Healthy Steps for Older Adults, further strengthening our Community Wellness offerings.

Through his leadership, initiative, and commitment to program quality, Rolando continues to strengthen our impact across Los Angeles County.
