Partners in Care Foundation We are innovators in the social and behavioral determinants of health. Partners is proud to be an LGBTQ friendly organization.

Partners collaborates with physician networks, health plans, community-based organizations, and federal and state agencies to deliver programs and services that protect and support adults with complex health and social services needs, frail elders, people with disabilities, caregivers and families. Our programs have been demonstrated to significantly reduce costly hospital re-admissions, ED visits, and nursing home placements, resulting in improved health outcomes at lower cost through better coordinated care.

Thank you to AARP for hosting an excellent “Tomorrow’s Caregiving Today” event at California State University, Los Angeles. In partnership with the Geriatric Social Work Education Consortium (GSWEC), the event highlighted the vital role of caregiving for older adults and the need for expanded resources and a stronger workforce. It also reflected the important work GSWEC is doing to prepare MSW students for careers serving our growing older adult population.

congratulations to Brenden Lanzanas for receiving Employee of the Month for November 2025.

Brenden is an exceptional member of the Partners team who first joined in 2024 as a temporary employee supporting the STSS and Private Duty Teams, where he quickly demonstrated leadership, initiative, and a strong desire to grow his skills. On January 1, 2025, he officially joined the Private Duty Team as a Community Support Coordinator and has since played a key role in advancing team goals and improving operations.

Brendan’s collaborative approach and dedication have greatly supported the Private Duty program’s progress toward achieving its performance metrics, and he was instrumental in the successful transition to Salesforce 2.0, helping to streamline workflows and enhance customer service for both members and partners. His professionalism and responsiveness have earned consistent praise from Home Care Organization partners and Managed Care plan leads, and he manages a high volume of referrals with impressive efficiency, ensuring timely follow-ups and thorough communication. Always willing to go the extra mile—often processing last-minute referrals late in the day or at week’s end—Brendan exemplifies commitment to member care and team success.

Olivia has become a cornerstone of our program’s success, handling a wide range of responsibilities with skill, care, and genuine compassion for the participants we serve. She manages participant intakes and eligibility verifications with exceptional attention to detail, ensuring a smooth and welcoming enrollment process that helps participants feel valued from the very beginning of their MSSP journey. She also plays a vital role in maintaining daily operations by supporting MSSP filings, managing documentation, and ensuring the Kern office runs efficiently. Over the past two decades, Rolfe has led HR strategy across healthcare, nonprofits, tech, and international organizations. Please join us in recognizing Herlinda Marin, Social Work Care Manager for our Home and Community-Based Alternatives (HCBA) Waiver Program, who is celebrating an outstanding 17 years with us this month!

Care Managers' work is vital to our mission. Through her compassionate care and deep commitment, she ensures that participants with the highest needs receive the services and support necessary to remain safely at home, avoiding unnecessary hospitalizations and nursing facility placements.
