Dementia Care Aware's tools integrate with the “4Ms” of the Age-Friendly Health System.

Our approach to screening, the cognitive health assessment (CHA), is brief and adaptable for a busy primary care setting. The CHA includes three steps:

1. A brief question about memory or thinking changes.

2. Use validated screening tools to assess cognition and function.

3. Identify if there is a care partner and document their information.
