Earlier this month, Chief Executive Officer Min Chang represented at the Mayoral State of the City Address in San Francisco.
Homebridge offers holistic solutions to meet growing demand for long-term care while addressing the shrinking workforce through development and training. Last year (and in alignment with Mayor Lurie's "Breaking the Cycle" initiative), we launched our "Street-to-Shelter-to-Home" model - creating a critical continuum of care by deploying teams directly in shelters to provide integral resources that break the cycle of homelessness and addiction.
Echoing these sentiments, we are inspired and empowered, looking forward to continuing to champion change and foster resilience. Together, we will advocate for all Californians who are unable to care for themselves to live independently at home through quality care and trained caregivers.

That’s how Jerry describes himself after losing nearly everything.
Once a carpenter and heavy-equipment operator, he raised four kids and built a good life on 40 acres in Lake County. Then the fires came and wiped it all out. His wife now lives with their daughter, battling Alzheimer’s. His sons don’t know he’s homeless.
Since the fires, Jerry has survived four heart attacks, three strokes, open-heart surgery, and a spinal disease that left him in a wheelchair. He’s never used drugs or alcohol, yet he falls through every system. As a Homebridge client, he needs basics (like pants), but our Client Care Fund is running out.

We make independent living possible through exceptional home care solutions
Homebridge provides home care and support services in San Francisco county. Our largest program is home care, in which we deliver more than 500,000 hours of home care service to over 1,200 clients annually. We also operate a training program for personal caregivers, and transitional care program for those discharged from hospitals.
Our expertise is in working with clients who are ill, cognitively-impaired or have other issues that make it challenging for them to live independently in their homes. Our innovative, client-centered model of care integrates case management, operations and scheduling support, and caregiver training, and is designed to tailor to the personal, changing and frequently-intensive needs of our clients.
Our home care clients reflect the ethnic and racial diversity of the Bay Area and speak more than 10 languages and dialects. We serve adults across the age spectrum, with approximately 50% of our clients over the age of 65 and 10% over the age of 85.
For our home care program, clients are referred to us by the San Francisco Department of Aging and Adult Services (DAAS) In-Home Supportive Services Program. We hire, oversee and manage home care providers for these clients, and we also provide them with case management services.
Importantly, we also view the training of caregivers as an key component of our mission, and strive to provide opportunities for professional development to both our own caregiver employees at the Homebridge and the students that come to our training program. Our training program provides industry-leading training to more than 800 home caregivers each year at our state-of-the-art training facility.
