How do you know what s safe when you use compounded medications?

(semaglutide for example)

This is a common inquiry I often hear from my patients, and it's a question I encourage anyone using a compounded medication to raise.

In the realm of medicine, we frequently encounter a dichotomy between the conventional and the alternative, and this duality extends to the choice between regular and compounded pharmacies. Many times, physicians exhibit reluctance towards the latter, despite their evident benefits, both in terms of cost-effectiveness and tailoring treatments to individual patient needs. The typical concern expressed is, "Do you remember that incident with a contaminated batch of medication?" Yes, I do remember, which is precisely why I am meticulous in my selection and scrutiny of sources.

Nevertheless, while I acknowledge that the conventional pharmaceutical industry generally upholds stringent safety standards, it's essential to keep in mind that they are not immune to medication recalls, which occur frequently throughout each year. To illustrate this point, here's an article to provide you with some insight:

So, what do you do?

Here is how you know you are in good hands with me(or what to ask your other medical providers using compounding medications):

I work with pharmacies designated FDA Outsourcing Facilities , often referred to as a 503B outsourcing facility due to their role in providing "office use" or "office administered" medications. This distinction means a significant advancement in patient care and safety, introduced by the FDA.

These outsourcing facilities adhere to rigorous cGMP (current good manufacturing practices) guidelines, mirroring the standards followed by pharmaceutical manufacturers. Under the new regulations, outsourcing facilities are the exclusive pharmacies authorized to supply compounded medications for use in physicians' offices, clinics, and hospitals.

The stringent nature of these new regulations has led only a select few compounders in the United States to elevate their pharmacies to the coveted FDA outsourcing facility status. Safety protocols are paramount here, commencing even before the compounding process begins. Each clean room undergoes air and surface sampling, and every technician wears full gowns and undergoes fingertip testing before entering the compounding area.

Upon completion of a medication batch, it is placed in a dedicated quarantine zone while samples from the batch are dispatched for independent third-party laboratory testing. These independent lab tests encompass sterility, endotoxin levels, and potency assessments. Typically, this testing spans a 14-day period. Once the results confirm compliance, the medications are released from quarantine and can be dispensed to patients or physician/clinic offices. These procedures exemplify just a fraction of the stringent guidelines that render an outsourcing facility the safest source for obtaining medications. For more detailed information, you can refer to the FDA's Q&A page on Outsourcing Facility Registration.

There are so many other quality standards, but I hope this information will help you get started on the right path!
