Active Home Health, Hospice & Personal Care was created with the goal of improving the standard of patient care in the home setting and actively serving those we care for in their journey through life. The ownership consists of local registered nurses and healthcare administrators that recognized a need in the community and wanted to make a positive impact on patient care. Although there are many home health, hospice and personal care companies in Utah, we wanted to raise the bar through our 35 plus years of combined healthcare experience, excellence in patient service and care, and most importantly, placing patient care where it belongs, above profit.

Meet Ashton!
Ashton Christensen is joining Active as a PRN Home Health Admit Nurse. She is originally from Oregon and brings a strong background in wound care. Ashton has been wound care certified for 3 years and proudly says, “the nastier the better.” She loves wound care because you can clearly see progress and healing, and she finds that incredibly rewarding.
Ashton is excited to work in home health so she can get out into the community and feel more connected to the people she serves. She is adventurous and always up for trying something new. In her free time, she enjoys rock hounding for crystals, painting, fishing, and is a certified scuba diver. She is also a proud dog mom to Ravioli, her corgi and Aussie mix and has been married to her best friend, Andrew, for 6 years.
We are excited to welcome Ashton to the team and look forward to the great care she will provide.

Meet Sarah!

Sarah is joining our team as a full-time Home Health Registered Nurse serving Salt Lake County. A proud new nursing graduate, she’s thrilled to begin her nursing journey in home health, where she’s eager to grow her skills, build confidence and make a meaningful difference in patients’ lives.

Sarah loves spending time close to family, especially when it includes their many animals (including horses and a bunny)! Her superpower is compassion, and we know that heartfelt care will help her truly shine in her role.

Sarah enjoys the outdoors, learning to bake something delicious or watching crime shows. Her nephews bring her endless joy, and she recently celebrated a big life milestone getting married last year, which has been such a fun new chapter!

Say hello to our newest Home Health Nurse – Eilene! Eilene brings her medical oncology hospital nursing experience as well as years as a CNA in the home health and assisted living settings. Through these roles, Eilene has developed an ability to connect with and truly listen to her patients, making her an excellent nurse focused on tailoring care for each individual patient. Eilene is looking forward to the one-on-one interactions with patients that home health affords. A self-described “grandma at heart,” Eilene enjoys spending time baking cookies and treats, cross stitching, cuddling with her two cats, and dreaming of the day when she owns a herd of Highland Cows. We are lucky to have Eilene on our team and can’t wait for our patients to get to know her!

Yesterday we celebrated together at Active Home Health Hospice and Personal Care.

Thank you to everyone who joined our Christmas party. Great food. Thoughtful gifts. A competitive relay race. Even better teamwork.

Shoutout to our nurses, aides, therapists, case managers, office team, and leaders who jumped in and made it fun. You showed energy, trust, and collaboration.
