Active Home Health, Hospice & Personal Care was created with the goal of improving the standard of patient care in the home setting and actively serving those we care for in their journey through life. The ownership consists of local registered nurses and healthcare administrators that recognized a need in the community and wanted to make a positive impact on patient care. Although there are many

home health, hospice and personal care companies in Utah, we wanted to raise the bar through our 35 plus years of combined healthcare experience, excellence in patient service and care, and most importantly, placing patient care where it belongs, above profit.

Please say hello to our newest SL County Home Health nurse, Stefanie! Stefanie was inspired to become a nurse in part because of the kind nurses who helped her dad during a stressful hospital stay. Stefanie values helping others, so becoming a nurse is a natural extension of who she is. Stefanie loves to learn and is excited for all the growth opportunities being a Home Health nurse provides. In her free time, Stefanie enjoys hiking, camping, four-wheeling, and gardening with her family, which includes her cat Diablo and husky Koda. Stefanie hopes to travel and see the world. We hope she does too! We are fortunate to have Stefanie on our team and know that the patients she cares for will feel the same!

We had a great time celebrating Easter with our amazing partner buildings.

Thank you to all the staff who joined in and made it fun. Your support means a lot to our team.

We loved seeing the winners who found our hidden eggs. Congrats to all, including our very own nurse Jenny.

We wish we had photos from every building, but we appreciate every single one of you who participated.

Brent is joining our team as a full-time Admit Nurse. Brent was a General Contractor prior to becoming a nurse in 2009. Then he made a complete career change, because he was looking for something that made more of a difference in the community and the people living here. Brent brings 16 years of Home Health and Hospice experience. Brent enjoys playing the Trumpet, singing and performing in the local arts. You may have even seen him perform in a local Semi-professional Orchestra or at Salt Lake City’s local Playhouse Desert Star. He looks forward to continuing to make a difference in the community and patient’s lives. He also looks forward to being a dependable and reliable co-worker who can really show up for his team. We are so excited to welcome Brent to the Active team and look forward to all of the knowledge, compassion and amazing care he will provide!

Congratulations to Alyssa Clausen on her new role as Intake Coordinator!
She truly set the bar high as Care Coordinator and made a lasting impact on our clients and team. While she is stepping out of that role, we are lucky she isn’t going far.

We’re excited to share that Sydney, one of our amazing Home Health & Hospice CNAs, is moving into the Care Coordinator role! She brings 4 years of CNA Coordinator experience, 8 total years as a CNA, and 6 years in Home Health and Hospice, giving her a strong foundation in patient care and a deep appreciation for the teamwork that supports our community.
Sydney is thrilled to jump into this new role and is especially excited to get to know our CNAs on a more individual level. Her passion for helping others, supporting our caregivers, and ensuring patients receive the best possible care shines through in everything she does.
When she’s not caring for patients or supporting the team, you can find Sydney at home with her three dogs, who she absolutely adores. 🐶🐶🐶

Meet Ashton!
Ashton Christensen is joining Active as a PRN Home Health Admit Nurse. She is originally from Oregon and brings a strong background in wound care. Ashton has been wound care certified for 3 years and proudly says, “the nastier the better.” She loves wound care because you can clearly see progress and healing, and she finds that incredibly rewarding.
Ashton is excited to work in home health so she can get out into the community and feel more connected to the people she serves. She is adventurous and always up for trying something new. In her free time, she enjoys rock hounding for crystals, painting, fishing, and is a certified scuba diver. She is also a proud dog mom to Ravioli, her corgi and Aussie mix and has been married to her best friend, Andrew, for 6 years.
We are excited to welcome Ashton to the team and look forward to the great care she will provide.

Meet Sarah!

Sarah is joining our team as a full-time Home Health Registered Nurse serving Salt Lake County. A proud new nursing graduate, she’s thrilled to begin her nursing journey in home health, where she’s eager to grow her skills, build confidence and make a meaningful difference in patients’ lives.

Sarah loves spending time close to family, especially when it includes their many animals (including horses and a bunny)! Her superpower is compassion, and we know that heartfelt care will help her truly shine in her role.

Sarah enjoys the outdoors, learning to bake something delicious or watching crime shows. Her nephews bring her endless joy, and she recently celebrated a big life milestone getting married last year, which has been such a fun new chapter!

We are so excited to have Sarah on the team!!!

Say hello to our newest Home Health Nurse – Eilene! Eilene brings her medical oncology hospital nursing experience as well as years as a CNA in the home health and assisted living settings. Through these roles, Eilene has developed an ability to connect with and truly listen to her patients, making her an excellent nurse focused on tailoring care for each individual patient. Eilene is looking forward to the one-on-one interactions with patients that home health affords. A self-described “grandma at heart,” Eilene enjoys spending time baking cookies and treats, cross stitching, cuddling with her two cats, and dreaming of the day when she owns a herd of Highland Cows. We are lucky to have Eilene on our team and can’t wait for our patients to get to know her!

Today we had the privilege of doing a pinning ceremony for the veterans from both Lomond Peak Nursing & Rehabilitation and Crestwood Rehab and Nursing. Thank you to both facilities for opening your doors and creating space for this moment. Your support helped us honor people who gave so much.

Thank you to every veteran who joined us. You carried burdens that many never see. You protected your community with courage. You shaped the lives we enjoy today. We are grateful for your service and your sacrifice.
