The moment is already lost.
Most people think documentation is a time problem. It’s not. It’s a continuity problem. When a doctor finishes a consult and then comes back hours later to write notes, they are reconstructing a story from memory. Small details get lost. Language becomes generic. The note becomes something written for the system, not for the patient. Over time, this affects care quality, claim accuracy, and even trust within teams. The real opportunity is not just saving time. It’s capturing the clinical moment while it’s still real. That shift changes everything, from better notes to fewer back and forth with insurers.
