Collaboration is the heart of community care!
Today, we had the pleasure of welcoming Senior Connection to The Wisdom Center for a tour of our Adult Day Health Care (ADHC) program, a wonderful opportunity to strengthen collaboration and expand the support available to older adults and their families across the Central Coast.
At The Wisdom Center/California Care Management, our ADHC provides seniors with the care, connection, and engagement they need to stay healthy, active, and independent. From skilled nursing and therapeutic services to social activities and caregiver support, our goal is to help each participant thrive in a warm, community-centered environment.
Senior Connection, a program of the Central Coast Commission for Senior Citizens, plays an essential role in helping older adults and caregivers navigate local resources. Their team connects families to services like ADHC, ensuring seniors can access the right level of care when they need it most.
