Valley Oaks Post Acute We are local owned and operated. Our administrators are onsite every business day. Our emphasis is on the individual needs of each of our residents.

We are a 59 bed Medicare and MediCal certified skilled nursing facility, dedicated to providing quality care to both short term rehabilitative and long term residents. Our goal is for each resident to have optimal quality of life during his or her stay with us. To this end we provide physical, occupational, and speech therapies, as well as I.V therapy and enteral therapy. The restorative nursing program enables residents to maintain the therapy gains they have achieved. As more and more residents, many in their 80's and 90's, enter the care center with Alzheimer's and Dementia, it's harder for them to reconnect to their memories in a lucid way. But thanks to a program called Music and Memory, in which Country Oaks Care Center is a participant, the residents there have a powerful tool to help them do just that.
