Arthritic hips and knees are replaced all the time—but did you know that arthritic ankles can also be replaced? In fact, ankle replacements in the U.S. more than doubled last year, thanks in part to advances in ankle implants (prostheses).

Total ankle replacement surgery—also called ankle arthroplasty—involves replacing a damaged joint with an artificial joint. The procedure greatly improves function for people who cannot perform everyday activities without experiencing severe pain. Arthritis and previous injuries are the most common causes of this pain.

In the past, the gold standard for treating these problematic patients was a fusion, or arthrodesis, in which the joint is removed and the bones are fused, thereby taking away the pain but also rendering the ankle immobile.

The procedure takes surgeons at FLOFAC about an hour to perform. After making an incision on the front portion of the ankle, the surgeon removes the arthritic bone and cartilage from the tibia (leg portion of the ankle) and talus (foot portion of the ankle). The implant typically consists of two metal components and a plastic spacer that fits between those metal components. These are then inserted to form a new ankle joint.

What can you expect from a total ankle replacement? Patients enjoy vastly improved function of their ankle, with pain-free weightbearing and range of motion.

This contrasts sharply to the results achieved from fusion, which not only eliminates ankle movement but also increases the chances of developing arthritis in adjacent bones due to ankle immobility.

FLOFAC wins 3rd place in a poster competition at the annual American Society of Podiatric Surgeons annual meeting with a unique and effective way to treat chronic plantar fasciitis. With minimal incisions, not cutting the plantar fascia and immediate post-operative weightbearing we have found excellent outcomes with this technique. Dr. Cottom received his bachelor’s degree with honors from Michigan State University. In 2002 he graduated Magna Cum Laude from Scholl College / Finch University, in Chicago, Illinois. Subsequently, he completed a three-year surgical residency in reconstructive and trauma surgery of the foot and ankle at the Detroit Medical Center/Wayne State University, in Detroit, Michigan focusing in all areas of Foot, Ankle, and Leg Surgery. Dr. Cottom is also extensively trained in the conservative and surgical treatment of diabetic conditions affecting the lower extremity including limb salvage.

Dr. Cottom is honored to be one of only a few podiatric surgeons in the United States and the only in the state of Florida that has completed a 12 month surgical fellowship at the prestigious Orthopedic Foot and Ankle Center in Columbus, Ohio. This is a unique fellowship that trains both Podiatric and Orthopedic Surgeons. This advanced surgical training focused on foot and ankle trauma, reconstruction as well as total ankle replacement. This was completed in Columbus, Ohio under the direction of Gregory Berlet MD, Thomas Lee MD and Terrence Philbin DO and was affiliated with The Ohio State University. This is the same type of fellowship that MD/DO orthopedic surgeons complete if they wish to sub-specialize in the foot and ankle.
In 2005 he completed an AO International Trauma and Reconstructive Foot and Leg Fellowship under the direction of Dr. Sigvard Hansen MD in Seattle, Washington. Dr. Cottom also completed a Fellowship in Kurgan, Russia focusing on the Ilizarov technique for correction of acute trauma and complex deformities of the lower extremity in both adult and pediatric patients.

Dr. Cottom is currently the only Fellowship Trained Foot and Ankle Surgeon in the region who is trained on 10 FDA approved Total Ankle Replacements. Not every patient is a candidate for this type of treatment. During a short visit in the office all options can be explored.

Dr. Badell received his bachelor’s degree in psychology from Indiana University where he was recognized as an academic founder’s scholar. He then went on to complete a master’s degree in biomedical science where he finished in the top of his class. Dr. Badell then attended Rosalind Franklin University in Chicago and earned his doctorate in podiatric medicine and surgery. He then went on to complete a comprehensive three-year trauma, reconstructive, and elective foot and ankle surgery residency at Community Health Network in Indianapolis. While in residency, Dr. Badell was exposed to a wide variety of surgical techniques throughout the lower extremity including trauma, arthritis care, deformity correction and reconstruction, peripheral nerve surgery, and limb salvage among many others.

Following residency training, Dr. Badell was chosen for the highly coveted fellowship position with Florida Orthopedic Foot and Ankle Center. This fellowship extensively covered all aspects of lower extremity surgery with a focus on trauma, complex deformity correction, total ankle replacement, and minimally invasive/arthroscopic procedures.

Dr. Badell has also been involved in academic research from his undergraduate education in psychology continuing into the present day. He has published in national journals and has won awards for his research at both national and regional academic conferences.

Dr. Badell takes pride in thoroughly explaining and discussing all possible treatment options with his patients. He wants each patient to be active in the decision making process, and takes the time to create a treatment plan specific to each patient.
