Outreach Team Finishes the Year Strong! 🌟

As 2025 comes to a close, we want to celebrate the incredible dedication of our CenterPlace Health Outreach Team for wrapping up a year of tireless community service!

Our team was out in full force this month, ensuring our neighbors had access to essential resources and health information:

💙Sarasota County DOH Screening: We provided pop-up screenings and warm hand-offs for dozens of residents at the Sarasota County Department of Health, ensuring local families were connected to primary care before the holidays.

💙Chandlers Funeral Home Celebration: We were proud to partner with Chandlers Funeral Home to celebrate their 30th Anniversary at the "Day in the Park" event. It was a fantastic opportunity to connect with community members, share vital health education, and celebrate local legacy!

Thank you to our dedicated Outreach Team for being the compassionate face of CenterPlace Health and maintaining our presence in the heart of the community.
