I can tell you from experience… it often isn’t.

At almost every visit, I find:
• Medications that were stopped but still listed
• Incorrect doses
• Duplicate prescriptions
• Supplements that were never documented

And sometimes—combinations that shouldn’t be taken together.

This isn’t about blame.

Your medication list should be reviewed at every single visit—even if nothing has changed.
