ATTENTION TO ALL PATIENTS

To continue providing timely and efficient care, Docu Family Medicine Center will be implementing a new policy regarding completion of all forms. This change will take effect on January 1, 2026.

Forms presented at the time of a visit will be completed as a courtesy and at no charge. Forms submitted outside of a scheduled visit will be subject to a $30 fee per form and will be returned within 7 business days. Rush forms, needed in fewer than 3 business days, will be subject to a $40 fee per form. If a document requires notarization, there is an additional fee of $3.00.

To avoid fees and delays, we encourage you to bring any necessary forms to your visit.

To our friends, The Pappas Family,

We are so incredibly saddened by the loss of such a charismatic, fierce, larger than life, beautiful soul. We know there are no words that can truly ease the pain, but we hope you all know how much of an impact Nick made on so many, especially everyone here at DFMC.

Nick was truly one of a kind and as we liked to jokingly call him, a Greek God! He was strong, kind, magnetic and full of heart. You could feel his presence in any room, not just because of his energy, but because of the love he radiated, especially for his family. He adored you all so deeply and all you had to do was speak to him for just a moment and you would realize that! Nick didn’t just live, he left a legacy in the way he loved you, in the values he passed down to you and most of all, how he always showed up for and protected those he cared about.

You all carry so much of his light in you, and I hope, even in this heavy grief, you can feel how much he loved you and how proud he was of you all. You are all a reflection of him and we hope you feel a sense of pride knowing that. Nick lives on in you and everyone who was lucky enough to know him. We are all so blessed to have had him in our lives!

Sending you all our love and strength and wishing you moments of peace even amidst all the pain.
