Our Story

Pathways has been serving the Capital District for over 30 plus years and has nationally recognized rehabilitation services. We help patients and families rehabilitate and plan for their next placement, whether to home or to an appropriate setting.

We have had hundreds of successful discharges of highly acute and complex medical cases. There is no better place to come if you have had a major illness or accident. We are experts in getting people rehabilitated and setup with the services and care they need for the long and short term.
