The Last Mile Connect Care Pilot Program, a partnership between CommonSpirit Health (CHI Health), First Choice Integrations, All Ways Wireless, and T-Mobile, aims to deliver essential healthcare to individuals at home. Funded by a Federal Communications Commission (FCC) Grant, the program focuses on bridging connectivity gaps.
This initiative provides complimentary top-tier routers to ensure broadband connectivity in rural, Tribal, and underserved communities across the Nation. CommonSpirit Health (CHI Health) rural patient gain access to in-home, high-speed 5G or 4G broadband without charge through June 2026. Participants have the option to discontinue service at the end of June 2026 (or later, if the grant is extended), or they can subscribe to an affordable monthly plan with T-Mobile to maintain the service. The router is provided free of charge and requires no return.
The only requirements are: having an address in rural Nebraska, and being a patient or an employee of CHI Health - stop by and visit Erin in our Hospital Lobby for more information. She will be onsite Today until 6pm, Wednesday and Thursday 8am - 5pm, and Friday 8am - 4pm.
