๐ Position Overview
The Physical Therapist (PT) evaluates patientsโ capabilities and limitations, develops and implements individualized treatment plans, and coordinates care within the total rehabilitation program. This role provides hands-on, quality patient care while supervising PTAs and HHAs as needed.
๐ฉบ Key Responsibilities Include:
โข Evaluating patient condition and establishing treatment goals
โข Developing and administering personalized treatment plans
โข Supervising PTAs and HHAs per plan of care
โข Providing therapy services as outlined in the Plan of Care
โข Educating patients and families on home programs and adaptive equipment
โข Maintaining timely and accurate documentation
โข Coordinating total patient care and obtaining necessary service orders
โข Ensuring HIPAA compliance and patient confidentiality

Qualifications:
โข Licensed Physical Therapist
โข Minimum 1 year of PT experience preferred
โข Physical completed within 6 months prior to first patient contact
โข Current TB results (within last 12 months)
โข CPR Certification
โข Valid driverโs license & proof of auto liability insurance
