Peripheral Giant Cell Granuloma in a healthy 20-something person.

• Benign reactive lesion
• Occurs only on gingiva / edentulous ridge
• Red–purple or blue nodular mass
• Sessile or pedunculated
• May bleed easily
• Usually < 2 cm
• Associated with local irritation

DIFFERENTIAL DIAGNOSIS

• Pyogenic granuloma
• Peripheral ossifying fibroma
• Irritation fibroma
• Hemangioma
• Metastatic lesion (rare)

TREATMENT

• Complete surgical excision
• Curettage to periosteum / PDL
• Eliminate local irritants

Recurrence: ~5–10%
Prognosis: Excellent

After I removed the crown I sectioned the roots buccolingually. I used my offset spade on the mesial hoping to snap the roots apart but instead they flexed into each other which actually allowed both roots to be removed easily. In billiards it would be the equivalent of a slop shot; not the way I planned but the same outcome.

Too much torque is not good. We really get into the weeds with prosthetic and surgical implant failures and how much running room is best for implant restorations in episode 31
