Honoring Choices PNW Our vision is that everyone in the Pacific Northwest will receive care that honors personal values and goals at the end of life.

Pacific Northwest physicians and hospitals are working together to ensure your choices are honored at the end-of-life. The Washington State Hospital Association and the Washington State Medical Association are collaborating on HonoringChoicesPNW.org – a comprehensive website with resources about advance care planning and end of life care options. The website will provide important information for patients and loved ones as well as health care professionals. For Patients and Loved Ones
Honoring Choices Pacific Northwest will offer the region resources to help people create their own personal plan care at the end of life. Essential information will help people get started, have the conversation and make a plan. It will provide details on understanding care choices and sharing your plan with family and loved ones. For Health Care Professionals
Planning for the end of life can be challenging, and people often delay this important work. Starting the conversation about end-of-life planning with patients can be tough, but taking that step can give them hope and put them at ease for whatever may lie ahead. Physicians play a vital role in helping patients understand what treatment options are available, including risks and benefits, so they can make decisions about the care they want should they face a serious illness or injury. Honoring Choices Pacific Northwest has assembled a multitude of resources for physicians, nurses, social workers, chaplains and other health care professionals in Washington state, including:
• Toolkits to guide physicians through the conversations
• Seminars and training to get up-to-date on best practices for helping patients share their wishes for end-of-life care.
• Research and articles about the need for advance care planning, including the different processes and approaches.
