It’s easy to enroll, and there are no strings attached. If you’re already enrolled in TRICARE®Prime you can switch to USFHP by simply filling out the DD Form 2826 application. Your coverage switches over the day we receive your application. If you are an Active Duty Family Member and currently covered under TRICARE®Prime, you can switch to USFHP by calling 1-800-585-5883.
