This patient in her 50s is looking amazing 3 months after upper and lower eyelid surgery and a deep plane face and neck lift. She still has some swelling at her neck but there is already a massive change. She left me a very thoughtful Google review that shared a lot of profound insight into how the procedures have altered the way she interacts with the world that are sentiments many patients share. I copied it below because she stated it more eloquently than I ever could:

Where are the incisions for a brow lift? There are numerous ways to do a brow lift,but in this video I talk about the two most common types of brow lifts. One is called a tricophytic (hairline) browlift and the other uses hidden incisions behind the hairline in an endoscopic style approach. There are pros and cons to both types of brow lifts but for most of my patients I perform an endoscopic style brow to hide the incisions behind the hairline.

This patient in her 40s is healing well just 10 days after a deep plane face and neck lift and lower eyelid surgery with fat grafting under her eyes. Typically swelling peaks about 2-3 days after surgery and then slowly gets better. Typically by the end of about 2 weeks the majority of bruising is gone but there may still be a few yellow patches where it hasn’t completely resolved. Swelling gets a lot better between week 1-2 but takes a few months before it fully resolves and the final shape can be seen. You can swipe to see how she looked before surgery, the day after surgery, and then at 10 days. You can see she still has some bruising at her neck and under her eyes but it has improved a lot from the day after surgery.

She is already thrilled with her results and I can’t wait to see the final outcome!

This patient in her 20s always hated having pictures taken from the side. She disliked her bump and overall feeling like her nose did not match her feminine facial features. She wanted to create a delicate swoop to her side view and also keep her tip from dropping when she smiles.

Immediate on the table results for a rhinoplasty to lower a small bump, support a droopy tip, and narrow a boxy wide tip. The supratip break always fills in with time and the tip tends to drop just a tiny bit so we need to exaggerate some of these features so that when it all softens over the next few months it looks just right. This nose should settle and have a perfect feminine transition from the tip to the rest of the nose once it’s all healed!

This patient in her 50s wanted to address her under eye bags. These bags develop when the connective tissue in the lower eyelid becomes weak (either genetically or with aging) which allows the fat around the eye to push forward into the eyelid

Unfortunately this is genetic fat that doesn’t respond at all to weight loss (actually sometimes looks WORSE with weight loss).

She is now 3 months out from a lower blepharoplasty (lower eyelid surgery) with fat repositioning and loves her results. During this procedure, rather than remove the bags, I relocated them down to her lid-cheek junction to fill in the tear trough and under eye hollowing.

This is a short outpatient procedure with about 1 week of down time.

Many people are nervous they will look “stretched” or that their mouth will look abnormal after a facelift. This typically results from poor surgical technique and a failure to understand and fully address all of the layers involved in facial aging. Fortunately this situation is completely avoidable and I have never had a patient tell me they look stretched or “windswept” after a facelift. A well done deep plane facelift should be natural and once the scars have faded nobody should be able to tell you had surgery. You should look exactly like yourself, just a more rejuvenated version of it!

This patient in her 40’s wanted to address what she called her “double chin.” She opted for a deep plane neck lift and is now 1 year out from her procedure. I love seeing how my results hold up down the road. This patients neck looks just as defined at 1 year as it did at 3 months which is exactly what I hope for all of my results!

If you are planning on having a facelift in the next few years there are a few aesthetic treatments that can make your surgery more difficult and potentially more risky:

1) Heat or ultrasound-based skin tightening treatments like thermage or ultherapy

2) Biostimulatory injectables like sculptra or radiesse to the cheeks or jawline. Injections in the temples are typically not problematic

3) Thread lifts - in addition to being very ineffective, they create scar tissue

Treatments that you can and should continue include good skin care (including retinol), laser treatments, microneedling, neuromodulators (like Botox)

Dermal fillers to the lips don’t cause problems (unless you are also interested in a lip lift) and can enhance a rejuvenation surgery. A conservative amount of subtle filler to the cheeks, temples, jawline, chin is usually not an issue. However, if you know you want surgery soon and are interested in fat grafting as part of your facelift it’s typically best to just avoid placing much filler which can “muddy the waters” and might have to be dissolved if fat grafting is planned.

It’s also best to avoid under eye filler if you are thinking about facial rejuvenation surgery. Usually this area is best treated with fat grafting and I will typically dissolve under eye filler if it is present before surgery.

Many patients wonder if we will have to break your bones during rhinoplasty. The answer is that it depends on the goals of the rhinoplasty and each patient’s anatomy, but the good news is that this is a lot less scary than it sounds. If we do perform osteotomies (breaks/cuts in bones) you may have a little worse bruising but it doesn’t make the pain any worse so it’s not something to be afraid of as a patient!
