Seronegative RA is a diagnosis of exclusion, which means you’ll need to rule out other diagnoses first. In this case, some of the other possible diagnoses we might consider are septic arthritis and gout. But approximately 17% of people with unilateral joint swelling do progress into rheumatoid arthritis, even though it’s atypical for RA.

How many markers get tested in an ANA depends completely on what your provider orders, but it averages anywhere from 7 to 11 markers on most tests. If it’s less than that, talk to your provider about what they’re ordering (and why!) to make sure you’re on the same page about what they’re looking for.

Autoimmune diseases LOVE to hang out together, especially if they’re diseases that have similar genetic patterns — that’s why co-occurrences of autoimmune diseases are so common. If you already have an autoimmune diagnosis and are dealing with new or different symptoms, speak with your provider or see a specialist in a different area to help you figure out if you were possibly misdiagnosed initially, or if you have a whole new disease flaring up.

Sjogren’s and lupus commonly co-occur, so while lupus may not necessarily be present now, there’s still a good chance it can develop in the future. Work with your provider to track your complement levels, lupus antibodies, and symptoms so you can be ready if and when lupus shows up!

An ESR and an ANA alone are not thorough enough to figure out why you’re experiencing these symptoms. Dr. Stocker talks about what testing you’ll need to get to the bottom of this, and what specifically you might be looking for from an infectious vs. autoimmune perspective.

A high ESR alone unfortunately doesn’t give us enough info to know what’s going on, but it’s a definite indicator of inflammation. Our recommendation is to work on lowering your ESR temporarily, which can also help manage your other symptoms. Then, work with your provider to do a deeper dive into your labs to find the root cause of your inflammation and pain. Did you know that if you’re dealing with POTS and an autoimmune disease, your inflammation can actually exacerbate your POTS symptoms? There are ways to manage your POTS symptoms separately, from beta blockers to compression socks to magnesium, but our number one tip for people living with both POTS and an autoimmune disease? Manage your autoimmune flares so they’re not acting as additional POTS triggers.
