Effective January 1st, deductibles are reinstated. It is essential to settle deductibles before your insurance policy covers office visits, regardless of the doctor's office. Additionally, several secondary Horizon Medicare plans have been discontinued. To verify your coverage, please provide us with your new insurance cards.
