Patients of Shaftsbury Medical will be getting a copy of this letter by mail in the upcoming days. As of June 30th, 2022, Shaftsbury Medical Associates will close be closing our doors.

" Dear Patients of Shaftsbury Medical,
I am writing this letter to inform you that after 36 years of serving the Shaftsbury community I have decided to retire, and the Shaftsbury Medical office will be closing as of June 30, 2022. We will continue to provide medical care for you until this closing date.

Future medical record requests can be sent to Shaftsbury Medical Associates at
PO Box 379, Shaftsbury, VT 05262. Requests will be filled as per time limitations dictated by state law retention requirements.

Shaftsbury Medical is happy to announce that Melissa Prouty APRN, FNP-BC has joined our practice.
Melissa comes from over 11 years’ experience serving our local community as a Primary Care Nurse Practitioner. She is Certified by the American board of Nurse Practitioners.
Her special interest includes Family Medicine, Women’s health, Geriatrics, and Palliative Care.
On her free time, she enjoys spending her time with her family boating and traveling.
She will be working Tuesday, Wednesday, and Thursdays from 9am to 5pm.
