It is with a heavy heart that we are announcing the closure of our practice in Sherman. It has been a distinct honor to participate in your health care. During our 8 years of practice in the Texoma region, we have made improving Women’s Health our number one priority. It has been our privilege to help deliver well over a thousand infants in the Texoma region.

Our practice excelled at providing compassionate care for many high-risk pregnancies. We have been at our patient’s side through the best and worst of times. The remarkable success of our practice has always been because we treat patients like our own family.

Consequently, this decision has not been easy.
As of 12/31/18, we will no longer be seeing patients. We will deliver patients through the end of December. We recommend seeking a new physician as soon as possible. Complete a release of information form using one of these methods:
a. Access form online by going to our website alexanderobgyn.com and clicking on “Records Release Form.”
b. Your new provider should have release of information forms that you can get from their office to send to us.
We will not release records based on verbal requests.

Fax or bring the form to our office at: (903) 892-8444

DO NOT send your form through EMAIL or facebook. This is not HIPAA compliant and will be destroyed if received through email or facebook.

The following fees apply for records:
Fax – Free
Hard Copy: Mailed - $25 for first 20 pages, 50¢ each additional page
+ $10 shipping & handling

Requests will be fulfilled in the order they are received, we have up to 15 business days to furnish your records after payment has been received. If you have not received your records two weeks after you have requested them, please contact our office for an update. (903) 892-8222. After 12/31/18, our office number will be turned off. We will keep our website alexanderobgyn.com running with updates on how you can contact us for medical records.
