Medical case management is a proactive process in which highly-skilled registered nurses collaborate closely with the patient, family, health care providers, and payor to identify and achieve functional and timely outcomes. This is done by coordinating health care resources, as well as maximizing efficiency, effectiveness, and communication. SCM has developed this process based on the standards of the Case Management Society of America.
