JOB DESCRIPTION:
• Charge Nurse: Long Term Care/SNF Facility
COMPENSATION:
• Competitive Market Salary based on experience
• Sign on Bonus based on license type.
POSITION SUMMARY:
• To provide nursing care to residents as prescribed by the physician and in accordance with standards of nursing practices, regulations and directed by the Director of Nursing. To lead or direct licensed and non-professional staff in the delivery of direct resident care and support functions.
QUALIFICATIONS:
• Must have a current LVN/RN license
• CPR certification required
• Ability to understand, remember and carry out verbal or written instructions in English
• Ability to complete assignments in a timely manner
• Ability to be flexible and positive with intermittent interruptions
ESSENTIAL FUNCTIONS:
• Current Texas LVN/RN license.
• To ensure resident safety.
• Provides the best possible nursing care by planning, organizing, and directing the nursing functions of patients in the unit
• Make nursing assignments appropriate to the skill level of employees
• Maintain acceptable standards of patient care
• Identify problems and guides personnel to their solution
• Create a working climate that provides growth and job satisfaction of personnel
• Accurately and promptly implement physicians orders
• Support and enforce infection-control policies and procedures
• Maintain a professional approach with confidentiality
• Cooperate and maintain good rapport with nursing staff, medical staff, other departments, and visitors
• Obtain and document nursing history from the patient and/or significant other that is comprehensive of the present condition and prior problems
• Perform basic assessment of the patient, indicating adequate knowledge of anatomy, physiology, and pathology
• Record vital signs notes changes and pursue more-specific investigation as needed
• Assess for and notify physician and other appropriate parties of changes in condition
• Use assessment information to develop a care plan before the end of duty time that communicates enough information for incoming personnel to adequately care for the patient
• Use care plans as a basis for providing safe and therapeutic care to patients
• Consistently follow established standards, policies, and procedures in providing nursing care
• Accept accountability for clinical care of assigned patients, including supervision of nonprofessional personnel
• Communicate with the patient and his or her family and other members of the health care team in carrying out the goals of the care plan
• Perform competently those technical skills considered basic to the type of care given in that unit
• Respond appropriately in urgent and/or emergency situations
• Support standards of nursing care through adherence to existing policies and procedures
• Seek guidance when encountering nursing procedures and responsibilities that have not been mastered
• Promote positive public relations with patients, residents, family members, and guests
• Use tactful, appropriate communications in sensitive and emotional situations
• Use appropriate workplace behavior and adhere to dress code at all times
• Utilize electronic medical record charting; Point Click Care
PHYSICAL REQUIREMENTS:
• Have the ability to safely perform movements such as pushing, pulling, lifting, bending, kneeling, reaching, and lifting up to 50 pounds with or without reasonable accommodations

• Certified Medication Aide
COMPENSATION:
• Competitive market salary based on experience
• Sign on bonus $5,000
SHIFTS AVAILABLE:
• 6a-2p Monday - Friday
• 2p-10p Monday - Friday
ESSENTIAL DUTIES AND RESPONSIBILITIES:
• Responsible for dispensing oral and topical medications to residents per physician’s order and properly document
• Observe residents for symptoms and conditions that could be related to medication interactions, adverse drug reactions or medication side effects and report any problems to the nurse
• Record what medications were given for the day and the dosage in the nurse’s medication administration record (MAR) chart
• Monitors medications to ensure adequate accountability measures are taken when medications are ordered, received into the facility, administered and at change of shift as required by policies and procedures and applicable state/federal regulations
• Demonstrates awareness of proper infection control practice by using Universal Precautions and following policies and procedures; including OSHA’s Blood borne Pathogen Standards
• Assist with resident care as needed.
EDUCATION & EXPERIENCE:
• Completion of training and competency program approved by the State of Texas and holds a current Nurses’ Aide Certificate from the State of Texas.

What a fun time we had at our annual Easter Egg hunt. Thank you River Realty & Walmart - Silsbee for donating gift cards and prizes . Silsbee Oaks along with these sponsors were able to give away 34 baskets, $105 cash eggs, $100 Walmart gift cards and $145 restaurant gift cards. Our Dietary staff did a phenomenal job on all the sweet treats. Also, a big high five to our awesome bunny "Mr. McDuffie". Young or old you can never pass up an opportunity to hug the one who brings the goodies. We're looking forward to doing it all again next year.
