This week, we’re celebrating 𝗣𝗮𝘁𝗶𝗲𝗻𝘁 𝗔𝗰𝗰𝗲𝘀𝘀 𝗪𝗲𝗲𝗸 and the incredible team members who help our patients. 💚

Our call center team and financial counselors are often the first connection patients have with Promise. From scheduling appointments to answering questions and helping navigate the financial side of care, they bring clarity, compassion, and support every step of the way. We’re so grateful for the care and patience they show our patients each day!

We are incredibly proud to share that our Hypertension Control Program has been featured in the Rural Health Models & Innovations collection by the Rural Health Information Hub! This national recognition highlights the innovative work our team is doing right here in Sioux Center to improve heart health outcomes across northwest Iowa.

We know that rural healthcare comes with unique challenges like distance and financial barriers. That’s why we offer solutions like our transportation program, free follow-up visits, and loaner blood pressure cuffs to make sure every patient can access care. By focusing on these barriers and implementing a team-based approach, including nurse health coaching, remote monitoring, and bilingual services, we’ve seen our blood pressure control rate jump from 𝟳𝟯% 𝗶𝗻 𝟮𝟬𝟮𝟮 𝘁𝗼 𝟴𝟰% 𝗶𝗻 𝟮𝟬𝟮𝟰! 📈👏
