After April 30, 2020, our office will be closed. To request medical records after that date please email us at officemanager@davisiwc.net to request a record release form or send a record release form. This email is confidential. All records will be sent out within 30 days from the request. Electronic transfer of records (encrypted) will require an email at a medical office to receive the information. Records can be sent electronically via email or paper copies to you for a small charge.
