Horizon Family Medicine in Smithfield will transition to UNC Physicians Network after June 27, 2021, but our address and phone will be the same. You can continue to enjoy health services at 410 Canterbury Road in Smithfield by scheduling a tentative 'place holder' appointment time. With your permission, we will convey your personal health information to UNCPN who will schedule your actual appointment. Drs. Pittard, Liverman, and Ryan and Jenah Manzano, PA will be available for patient care by July 1, and UNCPN anticipates adding other physicians to care for Dr. Davis's patients.

If you would like to continue your health care at 410 Canterbury Road, Smithfield, please drop by and sign personal health information release forms as soon as possible.

We have enjoyed serving you as Horizon Family Medicine for 40 years and believe UNCPN will be equally committed to ensuring continuity of care for our patients and our community.

Dr. Rhonda Davis will be retiring this summer. She will offer regular follow-up visits, wellness and urgent care through June 20, 2021, and offer limited services the following week. Please call if you need an appointment.
If you are a patient of Dr. Davis and have not been individually notified, please let us know. Also, if you would like to continue your health care at 410 Canterbury Road after she retires, please come by the office and sign a medical record release form as soon as possible.

Avoid holiday travel and limit physical contact with people who do not live with you.

If you choose to travel or gather with people who live outside of your household, follow these recommendations.
Get tested for COVID-19 three to four days before traveling or attending gatherings.
Live together? Sit together. If not, stay 6 feet apart. WEAR MASKS.
Do not host or attend a gathering if you’ve been exposed to COVID-19 or have symptoms.
Use single-serve options or have one person serve food so that multiple people are not handling items. (Do not sit close to or across from others who do no live in your household while eating or talking unmasked.)
Guests who are high risk for complications from COVID-19 should attend events virtually.
Clean and disinfect commonly touched surfaces such as door handles, sink faucets, remotes.
Keep the guest list small.
Host your gathering outdoors. If that’s not possible, make sure the room is well-ventilated by opening windows and doors.

For your family’s acute care needs, Smithfield offers walk-in hours 8:30-9:30 Monday through Friday. We also offer our Acute Care Clinic (ACC) on Saturday mornings in Clayton and Sunday afternoons in Smithfield. (Check with the office to confirm holiday weekend hours).
