Dhar Family Medicine has received PCMH re-certification for 2024. We are among the top 20% of the U.S. Primary Care Providers with PCMH Certification by National Committee for Quality Assurance (NCQA), Washington DC. The goal of the PCMH model in the USA is to provide safe, high-quality, affordable, and accessible patient-centered care. Thank you to the staff members for their teamwork, from front desk to back end nurses, including providers, ITs and EMR who works in the background and has been helping to sustain PCMH Level-3 status since the year 2010!

A year ago if someone told me I can hike Grand Canyon, I would have never believed it. I found a passionate group of hiking people and decided to try. Slowly.

I got into the experience of hiking weekly and it helped with my self-discipline and endurance; I truly started enjoying the whole experience. I have just completed hiking the Grand Canyon rim-to-rim and it was an amazing, humbling, and out of the world experience. I accomplished this feat at 54 years of age- an experience which would never have crossed my dreams in my 30's.

I am posting few pictures specifically for my wonderful patients. I caution that I am not at all personally active on Facebook. However, if you are interested, I can give you all the tips from my experience at our next appointment. We are all capable of doing anything if we are determined. “The adage of game is won in the minds of the player” now makes complete sense to me.

This was long awaited and now there is a CDC Guidelines on Diabetes and Hearing Loss Connection: “ CDC now recommends to get hearing evaluation every year if you have Diabetes, similar to vision or foot exam. Over time, blood sugar levels that are too high or too low can damage nerves that affect your hearing. Learn how you can help prevent hearing loss if you have diabetes”. This will be added on DFM and Audiology Consultants websites under patient education.
