Beginning on October 1st, our office & staff will be moving back to Prompt Care, Inc. in Daniels.
Starting in September, Faith will only be open Wednesdays & Thursdays from 8-2.
We count it a blessing and a honor to have cared for our family practice and urgent care patient at Lester Square. Samantha will be working at both Prompt Care and Faith the month of September and starting October 1st she will be full time at Prompt Care. Sam will continue to see family practice and urgent care patients at Prompt Care.
We will be sending letters out to our family practice patients in regards to our transition.

Our team has truly loved caring for you over the past 3 years. We hope to see you at our new location.
