While often grouped together, different vascular conditions may require different lasers or laser-like devices to achieve best clearance. Shorter wavelengths, such as that of the KTP laser, may be best suited for superficial facial veins. Port-wine stains typically respond best to pulsed-dye laser treatments. On the other hand, longer wavelengths (such as that of the Nd:YAG laser) may pe*****te deeper, allowing for the treatment of deeper leg veins. Nd:YAG laser is also safer for the treatment of vascular lesions in patients with darker skin tone. Intense pulsed light (IPL), a broadband, laser-like device, may work best for diffuse redness associated with rosacea. Thus, comprehensive knowledge in the field of laser surgery is of the utmost significance when selecting a laser practitioner.

Scars that are elevated can be subdivided into hypertrophic scars and keloids.
Hypertrophic scars are prominent, firm, raised scars that often occur in areas of high tension, such as the upper back, chest, and shoulders, most commonly after surgery at these sites. These scars stay inside the margins of the original surgery or trauma wound that caused it.
Keloids are more even more prominent and outgrow the margins of the original wound. These may occur on the face, ears (such as after ear piercing), trunk, arms, and other locations and may even occur spontaneously, without previous trauma. Keloids are more common in African-Americans, younger individuals, and those with a family history of keloid formation.
Both types of scars can be associated with pain, itching, and discoloration. Surgery to remove these scars, especially in the case of keloids, without additional measures, such as steroid injections, will often result in the recurrence of the scar, sometimes even larger in size. Several effective treatment options exist, including injection of steroid or chemotherapeutic agents into the scar tissue, compression devices, and even radiation therapy.

Telangiectasias, commonly referred to as broken capillaries, are often found on the face and, especially, on the nose. They are typically related to an underlying diagnosis of rosacea or excessive cumulative sun exposure. While cosmetically bothersome, telangiectasias are usually quite easy to treat with lasers, such as the KTP laser. Several sessions may be required for complete removal. Mild redness immediately following the treatment is typical, but disappears within a few hours.

Venous lakes are common purplish vascular lesions, typically found on the lower lip or, less frequently, on the upper lip or the ear. They are thought to arise as a result of excessive sun exposure. Venous lakes can be treated using a vascular laser with excellent results, often requiring 1 to 2 treatment sessions.

Actinic keratoses, commonly called "pre-cancers", usually present as red or tan scaly rough patches. Often confused with dry skin, these occur on sun-damaged areas and do not resolve with topical emollients. Although the risk of conversion of each individual actinic keratosis to skin cancer (typically squamous cell carcinoma, seen in the middle of the photograph) is low, the risk is significantly higher when multiple lesions are present, such as in this case. Common treatments include cryosurgery ("freezing") with liquid nitrogen, topical chemotherapy or immunomodulatory agents, and photodynamic therapy.

New study by Dr. Teo Soleymani et al published by the Journal of the American Academy of Dermatology confirmed lower rates of local recurrence, metastases, and disease-specific mortality in high-risk squamous cell carcinomas of the skin following treatment with Mohs micrographic surgery as compared to wide local excision.

Hands are often neglected in the overall approach to rejuvenation. Aging hands may show discoloration in the form of freckles or sun spots, as well as lost volume resulting in prominent tendons and veins. In this case, freckles and other signs of chronic sun damage were treated with an intense pulsed device (IPL), a laser-like device best suited for large surfaces, such as that of the back of the hands.

Some skin cancers occur in the so-called "danger zones", where the risk of damage to the underlying anatomical structures is elevated due to their superficial position in the skin. This large squamous cell carcinoma is overlying the path of the temporal branch of the facial nerve (marked as a broken line) in a location where the nerve is very superficial. Thus, the risk of damage to the nerve during surgery is very high. Such damage would result in same-side eyebrow droop and inability to raise the eyebrow, as well as partial upper eyelid droop.
Mohs surgery is the best treatment modality for this cancer at this location; however, if the cancer is found around the nerve, post-operative radiation therapy would also be recommended.

A recurrent skin cancer is one that returns at the same or immediately adjacent location after being treated. While most recurrences happen within 1 year of the original treatment, some may take years to manifest. Scar tissue often hinders the ability to see the full extent of the cancer. This recurrent basal cell carcinoma continued to grow underneath the scar from a previous procedure and finally manifested itself as a non-healing ulceration just above it. Mohs surgery is typically the best treatment modality for recurrent skin cancers.
