After a fall that resulted in fractures to her left humerus and right patella, Verla moved into Longevity of Granite City to begin her recovery. Despite starting in a manual wheelchair with a knee immobilizer and arm sling, she showed incredible determination.

With the support of PT Paulina Milanowski, Verla progressed from walking short distances with a hemi walker to walking community distances with no assistive device. Her hard work—along with the constant encouragement from her daughter and granddaughter—made all the difference.

When a resident began experiencing dizziness, fatigue, and a lingering leg infection, the EmpowerMe Team acted quickly. Because provides a daily on-site clinical team, elevated blood pressure was identified during a routine check-in and her provider was notified immediately.

After 24 hours of close monitoring, her P*P scheduled an appointment—leading to an ER referral where she received timely treatment without being admitted and returned home the same day.

Today, the team continues to monitor her vitals, coordinate with her provider, and support her through cardiology follow-up. With EmpowerMe 360’s coordinated approach, she’s safely thriving in her community.

When the Clinic Director at Reflections Memory Care reached out to EmpowerMe Medical Group for urgent help for a resident on a Friday afternoon, our Medical Group jumped into action. Our team quickly evaluated the resident, consulted with a specialist, adjusted medications, and personally called the resident’s daughter to share the care plan.

Thanks to this rapid, coordinated response, the resident avoided further ER visits, regained appetite, and became more engaged in the community.
