Managing medications shouldn’t feel like a full-time job (or a constant source of anxiety). Our Care Manager, Patti Cox, recently shared why she’s so passionate about medication management—it’s not just about organization; it’s about safety and peace of mind.

Between complex schedules and post-hospitalization changes, the risk of errors is real. Patti’s expertise and tools like mediplanners help reduce hospitalizations and give families the confidence they need.

Struggling to keep track of a loved one’s medications? We’re here to help you simplify the process and keep them safe at home.

Learning the New Science of Connection!

We constantly look for ways to transform every interaction to help move our clients from survive to thrive. Care Choice Care Management hosted a seminar that provided neuroscience-based tools for trust, change and better outcomes.

Workshop Highlights

Part 1. Beyond “Rapport”: Where Compassion Meets Effectiveness. See beyond bias to understand clients’ hopes, dreams, and fears and break down “resistance barriers” to create genuine cooperation.

Part 2. Move Clients from Victim to Victor. A new, neuroscience-based model of behavior change. Clients see for themselves that they can create new behaviors. Clients learn how to build motivation from within despite stressful situations

The presenters were:

Polly Lemire Certified Behavior Design Specialist & Tiny Habits Coach; trained in Stanford University’s Behavior Design Lab. Certified in NLP for business communication. Former corporate trainer and marketing professional dedicated to helping people create behaviors that make life glow with confidence and possibility.

Sue Haverman Certified Habit Finder Coach and experienced workforce development professional with 20+ years in nonprofits, case management, and teaching along with building employment-readiness and skill-building programs.

One of our Nurse Care Managers, Stacey, participated in this seminar and will present this information to our Care Supervisory Team.

💰 Can a Care Manager Actually Save You Money? 💰

When a loved one begins to need extra help, the first thing most families look at is the price tag. With assisted living costs in Missouri often exceeding $4,000 a month, it’s natural to feel overwhelmed, especially if your loved one is living on a fixed Social Security income.

You might think that hiring a Professional Care Manager is a luxury you can't afford. In reality, a Care Manager is often the key to avoiding the most expensive mistakes in senior care.

Here is how professional care management saves St. Louis families money in the long run:

1. Avoiding the "Crisis Move"
The most expensive way to choose senior living is during a crisis (like a fall or a sudden hospital discharge). When you’re rushed, you often choose the first place with an opening, which may be overpriced or lack the specific care your loved one needs. This leads to "re-moving" a few months later. A Care Manager helps you plan ahead, ensuring the first move is the right move, saving you thousands in move-in fees and unnecessary transitions.

2. Navigating Medicaid and "Hidden" Benefits
Many families believe that if they can’t afford private pay, their only option is a state-run nursing home. This isn't always true. In Missouri, there are programs like Supplemental Nursing Care (SNC) and specific Medicaid Waivers that can help cover costs in certain residential care settings. At Care Choice, we understand the "alphabet soup" of entitlements. We help you identify:

Veterans Aid & Attendance benefits that many families overlook.

Medicaid-eligible assisted living options in the St. Louis area.

Asset protection strategies by referring you to trusted Elder Law attorneys.

3. Preventing Hospital Readmissions
Hospital stays are incredibly costly, both financially and physically. Care Managers act as the "eyes and ears" on the ground. By monitoring medications, attending doctor appointments, and identifying safety risks at home before they lead to a fall, we help prevent the emergency room visits that drain savings and health.

4. Vetting Home Care Agencies
If your goal is to keep Mom at home, you may be tempted to hire the first home care agency you find. However, not all agencies are created equal. A Care Manager knows which local St. Louis agencies provide the best value and which "add-on" services you actually need, and which ones you don’t.

5. Saving the "Daughter/Son Track"
We often forget the "indirect" costs of caregiving. When a family member has to take time off work, miss promotions, or travel back and forth to St. Louis to manage a crisis, the financial impact is massive. We take the administrative burden off your shoulders, allowing you to stay focused on your career and your relationship with your parent.

Addressing Your Specific Situation: "Mom is on Social Security and needs help NOW."
If you are currently looking at your mom’s Social Security check and realizing it won't cover $4,000/month for assisted living, you are not alone. What can you do?

Look into Residential Care Facilities (RCFs): In Missouri, RCFs are often more affordable than "Assisted Living" but offer similar help with meds and meals. Some accept Medicaid or the SNC grant.

Explore Medicaid Waivers: Missouri has programs designed to keep seniors in the community rather than in a nursing home.

Consult a Professional: A one-time assessment with a Care Manager can give you a roadmap of exactly which St. Louis facilities accept Medicaid and how to apply for the Supplemental Nursing Care grant.

Don't navigate the complex world of Missouri senior care alone.

At Care Choice Care Management, we specialize in helping St. Louis families find cost-effective, high-quality solutions for their loved ones.
