Beautiful story of one of our residents, highlighting the challenges people with IDD encounter in getting the support they need, and the incredible determination and dedication shown by our partners in the Family Navigation Team from Sunnyhill Inc., UCP Heartland, and St. Louis Arc. And of course, the Productive Living Board, whose financial assistance is even more invaluable during these uncertain times.

After more than 20 years of independent living and steady employment, Mary Ellenโs life changed in April 2024 when a major health crisis made it unsafe for her to return home. She needed supportive housing, but a clerical error during Medicaid enrollment blocked access to the Department of Mental Health (DMH) funding she qualified for.
As discharge from rehab neared, she was at risk of being placed in a nursing facility over 60 miles away, isolating her from her job, community, and support network.
While state systems remained tied up in red tape, PLB-funded services provided a path forward. To avoid unnecessary institutionalization, Sunnyhill's Family Navigation team quickly secured a temporary placement at UCP Heartland's Oak Tree respite home, keeping Mary Ellen close to her community.
The Family Navigators worked tirelessly alongside Mary Ellenโs sister to challenge the error by resubmitting applications, making countless calls, and waiting in person at Family Support Division offices for hours at a time. By July, the Medicaid error was corrected, but by then, DMH had paused new funding approvals.
Another health setback in fall 2024 forced her into an overly restrictive skilled nursing facility, where her independence and well-being declined. Still, the Family Navigators persisted. This unnecessary institutionalization violated the Olmstead Act, which mandates community-based services for individuals with disabilities when appropriate and desired. Together, they filed for Mary Ellenโs transition into an Independent Supported Living (ISL) arrangement.
In April 2025, she moved into her permanent home through a partnership between Promise Community Homes and St. Louis Arc. Today, Mary Ellen is living with dignity, surrounded by support, and exploring new opportunities with her housemates and neighbors.
