We kindly ask for your patience as our office is currently receiving a very high volume of phone calls.
If you are calling for a prescription refill, please leave a voicemail with the following information:
β’ Your full name
β’ Date of birth
β’ The medication you need refilled
β’ Your pharmacy name
Please allow up to 48 hours for refills to be processed. Prescription refills will continue to be provided through June 5th.
We also want to let everyone know that very few appointments remain available at this time. However, if you are due to be seen but need a prescription refill, those prescriptions can still be refilled.
For medical records, once you decide where you would like your records transferred, please have your new provider fax us a signed medical records release form. You may also come into the office and sign a release form if you would like your records released directly to you.
