Join us in celebrating our amazing Early Intervention (EI) Case Managers Michelle Hoza and Sarah Grimes for the incredible work they do every day!

Our Horizons Early Intervention program serves children from birth to age 3 who have developmental delays or disabilities by coordinating and providing access to critical services and therapies including physical therapy, occupational therapy, speech therapy, and social-emotional therapy, etc. This is only possible with the dedication and compassion of our amazing case managers.

Michelle and Sarah work tirelessly to support families, coordinate services, and ensure that each child receives the resources they need to thrive during these critical early years.

This Case Management Week, we recognize the vital role they play in empowering families and helping young children reach their fullest potential. 💙
