United Community Health Center (UCHC) serves your medical, dental, and behavioral health needs. Welcome to the United Community Health Center (UCHC).

UCHC is one of only thirteen Federally Qualified Health Centers (FQHCs) within the State of Iowa. UCHC is one of fourteen Federally Qualified Health Centers (FQHCs) within the State of Iowa, primarily covering Buena Vista and Sac Counties. However, all patients are welcome, no matter where you live. We have been growing and developing within the Storm Lake region since we opened in March 2006. United Community Health Center is privileged to serve our patients and community using a culturally diverse and experienced team of clinicians and other professional staff. We are also proud to have productive community, state, local and national partnerships that continue to enhance our ability to perform our work and thus increase our capacity to bring resources to you. We are truly committed to provide comprehensive and culturally appropriate primary and preventive health care services for your entire family. Care received at UCHC is not free – it is extremely affordable. UCHC provides a way for patients to see a doctor on a sliding fee scale that doesn’t rely on insurance status, although we are very happy to serve insured patients as well. We encourage all people that are uninsured, underinsured, or insured seeking a Medical Home to consider us as your primary care provider. With busy schedules in mind, we have convenient hours of operation including evenings and weekends. Come visit us at 715 W. 🌟🎉 United Community Health is beyond proud to celebrate Sara Boysen as she completes her Post-Master’s Mental Health Certification with us! 🎉🌟

From May 13 to September 9, 2025, Sara dedicated over 180 clinical hours as a Psychiatric Mental Health Nurse Practitioner student. She truly went above and beyond, showing incredible compassion, dedication, and skill with every patient she served. 💙

As we send her off, we’re thrilled to see what’s next! Sara will continue her journey in Emmetsburg, where she will be dual-certified as both an FNP and PMHNP—an amazing accomplishment that will allow her to provide even more comprehensive care to her community.
