To qualify for care at A Community Clinic, you must meet the following requirements:
* You must live in Union, Snyder, Montour, Columbia or Northumberland County.
* Must be 19 - 64 years of age.
* Must have NO form of health insurance. (We do not accept CHIP, Medicaid, Medicare, GHP, Blue Cross/Blue Shield, Veterans Benefits or any other form of insurance.)

Documents Required for Eligibility Screening
* Income verification is required in order to determine the eligibility for free medication assistance and specialty referrals. Verification would be last year's income tax return, Social Security Retirement, Supplemental Security Income, Social Security Disability, Pension, Retirement or Unemployment Award Letter
* Each individual must provide some form of photo identification.
