My normal schedule is as follows:

Monday: 10-4
Tuesday: 6:45-4, morning walk-in from 6:45-8.
Wednesday: 8-4
Thursday: 8-6, walk-in 4-6 pm.

This month, I will be out of the office on March 11 and 12, as well as March 23 and 24. Over the next few months, I will be incorporating one Friday a month due to being off one Thursday a month, to ensure I will still be able to provide care to my patients.

Tenncare, Medicare, and most commercial insurances are accepted.

-Tuesday: 9:00-4:00
-Wednesday: 9:00-4:00
-Thursday: 9:00-4:00, 4-6pm walk-in clinic.
-Friday: off
-Saturday: 8:00- 11:00, every 6 week rotation.

We are currently experiencing an internet outage at the hospital which has our computers down at this time. They are working on the problem. If you are needing to be seen you can walk in for an appointment.

If you are a patient of Dr. Ken Kozawa or any of his providers, (Kellie Gallaher, FNP, Jacob Gallaher, PA-C, Renea Cagle, FNP, Michelle Davis, FNP, Dawn Frerichs, FNP, Thomas Vance, FNP), at Sweetwater Hospital Association, it has come to our attention that the United Healthcare patients currently being seen at this office are receiving letters 🗒️stating that this office is no longer taking their insurance. These letters are false causing confusion among patients. Please note that this provider clinic does take United Healthcare patients. Dr. Ken Kozawa and his providers look forward to continuing to care for their patients. 🩺 Again we apologize for any confusion.
