Cherokee Elder Care PACE

Cherokee Elder Care PACE Call us to learn more! What is a PACE program? All services are provided through Medicare and Medicaid.

PACE (Programs of All-inclusive Care for the Elderly) is a federal program designed to keep elders living in their homes, connected with their communities and out of nursing home facilities. The PACE center combines the services of an adult day health center, primary care office, and rehabilitation facility into a single location. Services include but are not limited to primary care, rehabilitation, prescription medication, meals/nutritional counseling, respite services, caregiver training, home health and transportation. Utilizing an inter-disciplinary team (IDT) comprised of physicians, registered nurses, nurse practitioners, therapists, social service workers, dieticians, and transportation specialists, the total needs of the elder can be addressed. This provides an all-inclusive and comprehensive continuum of care designed to maintain and ideally to improve the quality of life for our elderly. Inpatient services (temporary nursing home and hospital) and dental services are provided through partnerships with other providers. Cherokee Elder Care PACE Eligibility Requirements

Yesterday we toured W.W. Hastings Hospital! The Vanderbilt students had the opportunity to see a part of Cherokee Nation’s healthcare system and learn more about the services provided to the community.

They also got to see the beautiful artwork displayed throughout the hospital and hear about the important role healthcare plays within the Cherokee Nation. It was a great experience and a meaningful part of their visit!

Crafts?? Count us in! Our participants had a great time making **pinch pots**. You’re probably thinking, “what in the world is that?”

Pinch pots are a simple type of pottery made by **pinching and shaping clay with your fingers** instead of using a pottery wheel.

**How they’re made:**
• Start with a ball of clay.
• Press your thumb into the center to make a hole.
• Pinch the sides gently while turning the clay to thin and shape the walls.
• Keep pinching and smoothing until it forms a small bowl, cup, or pot.

It was a fun, hands-on activity and everyone enjoyed getting a little creative! 🤍

The Vanderbilt students helped out around a couple of our participants’ homes today 🤍 Sometimes it’s the little things that make someone’s day, and today they definitely did!

They’ve only been here for two days and have already made a huge difference. We are so grateful for their time, kindness, and willingness to serve our community!
