As we celebrate World Social Work Day we are especially thankful for our team of social workers at Healthmap Solutions. These individuals make tremendous impacts on our members’ health and wellbeing by identifying and addressing their social determinants of health. One of our social workers, Brittany Boeltz-Guerra, MSW, LSW, shared, “I feel fulfilled by helping individuals eliminate barriers by connecting them to needed behavioral health services, obtaining financial assistance, or signing up for transportation to get medical appointments. This can help keep our members safe, successful, and out of the hospital with an improved quality of life. I feel like I get to impact our members’ lives every single day, which is why I wanted to be a social worker.”
Thank you to Brittany and all of our social workers for helping our members get the support they need to live well!

March is Social Work Month. At Healthmap Solutions, we recognize that advancing healthcare equity means addressing more than clinical care alone. Social workers play a vital role in helping patients navigate barriers related to transportation, housing, food access, insurance, and health literacy. These social determinants of health can directly impact outcomes, especially for individuals living with kidney disease and other long-term, chronic conditions. It’s why social workers are a critical component of Healthmap’s multidisciplinary Care Navigation team. This month, we thank social workers for the essential support they provide to patients and communities every day. For more, check out the link below: https://hubs.li/Q045pXKy0
