Griswold Care Pairing for Hillsborough and Pasco counties is proud to announce that it is partnering with Healthspan Partners of SWFL as one of their Guide Home Care Respite providers

What is Guide?
Last year CMS introduced a new program to assist those with dementia.
“The GUIDE Model focuses on comprehensive, coordinated dementia care and aims to improve quality of life for people with dementia, reduce strain on their unpaid caregivers, and enable people with dementia to remain in their homes and communities.” It plans to achieve those goals “through Medicare payments for a comprehensive package of care coordination and care management, caregiver education and support, and respite services.”

To participate, an individual needs to:
Have a diagnosis of Dementia
be on traditional Medicare (not Medicare Advantage)
and, not be in a skilled nursing facility (ie needs to be living at home of in an ALF)

All participants get:
an array of dementia related resources
and a Navigator (Care Manager) to help them navigate access and maximize the benefit of the community resources available to them
If they have a caregiver that is assisting them (lives with them or assists them from afar), they could also qualify for $2500 a year in respite care (either through Home Care like Griswold Care Pairing or via attendance at an Adult Day Care).

Master providers are contracting with CMS to provide Guide services. Each one is creating their own network of Dementia services, Navigators (Care Managers), Home Care providers, Adult Day Care providers, etc. Areas can have multiple master providers. Griswold Care Pairing for Hillsborough and Pasco counties is proud to announce that it will be partnering with Healthspan Partners of SWFL as one of their Assessment providers and also one of their Home Care Respite providers. As far as we know, Healthspan is only one of two Master Providers in Hillsborough and Pasco at this time. And we will be one of only 3-4 Home Care providers in their network.

We are just getting started so we still have a lot to learn about what our network’s plan has to offer (besides us of course). We will share more as we get it.
